Restrictions: This form is for healthcare providers only. You must be a healthcare practitioner or a practice manager to request this phone call, or our standard consultation fees may apply (i.e., currently, $300 per hour, billed in 15-minute increments). Callbacks will be made during our regular office hours, except on national public holidays in the United States and Israel, seasonal equinoxes and solstices, and when our lead nutrition practitioner is out of the office. Please do not use this form to communicate protected health information (PHI) as it is not HIPAA compliant. For patient referrals, please use our secure patient referral form.
