Why We Ask for Your Original Lab Report

We use your original lab report to create a Functional Health Report — a powerful visual tool that goes beyond “normal” to help you understand what’s really going on in your body.

Unlike traditional lab reviews, this report:

✅ Highlights trends before they become problems
✅ Maps your results to optimal (not just “normal”) ranges
✅ Helps us personalize your nutrition and lifestyle plan with precision
✅ Gives you clear, trackable progress over time

To generate this report, we use secure software that requires a complete, unedited PDF from your lab’s portal (e.g., Quest or LabCorp). Uploading screenshots, MyChart summaries, or scans won’t work — and may result in a manual processing fee (currently $45/report).

Uploading the right file = faster insights, fewer delays, better care.

compatible lab right at the top.

What to Upload:

✅ Original PDF downloaded from your lab’s portal (e.g., Quest, LabCorp, other compatible lab)

✅ Includes your name, date of birth, provider name, test date, and full results

✅ All pages, in English, unedited

What to Avoid:

🚫 Screenshots or phone photos

🚫 Scanned paper printouts

🚫 MyChart or other client portal summaries

🚫 Combined or edited PDFs

How to Access Your LabCorp Report

  1. Log in: patient.labcorp.com

  2. Go to “Results

  3. Click “View” for your most recent blood test

  4. Choose “Download Official Report

  5. Download and save the file as a PDF

Heads up: If you received a MyChart or hospital portal summary, it’s not the right file. You’ll still need the full report from Quest, LabCorp, or the lab that processed your tests.

How to Access Your Quest Report

  1. Log in: myquest.questdiagnostics.com

  2. Go to “Results

  3. Click “” for your most recent blood test

  4. Choose “Print PDF

  5. Download and save the file as a PDF

  2. Go to My Resources > Documents

  3. Click Upload, select your saved PDF, and click Share
